Initial Oral Surgery Consultation

WHAT DO I BRING TO MY FIRST APPOINTMENT?

At your first appointment, it is important that we have all of the information necessary to plan and discuss your treatment. When we contact you to confirm your appointment, we will remind you of some of the things you need to bring.

The following things are important for your first visit:

  • Referral from your dentist/doctor
    Sometimes this has been sent to us already. If you are unsure, please contact us, and we will let you know if it has been received.

  • X–rays
    If you have been referred to our office by your dentist or another doctor, you may have already had a dental X-ray taken. Our staff will be happy to work with you and the referring office to get any appropriate X-rays transferred, or we can arrange for you to have new X-rays taken at the time of your consultation.

  • List of medications
    If you have an extensive medical history, we may request that you obtain a health summary printout from your primary care physician to bring to your appointment.

  • Insurance information
    If you have dental and/or medical insurance, please bring your insurance card to your first appointment.

WHAT CAN I EXPECT AT MY FIRST APPOINTMENT?

When you arrive, you will be checked in by the reception staff who will confirm all relevant details, including insurance information and medical history documentation. You are encouraged to provide this information to us online prior to your appointment by completing our Patient Registration form.

During your consultation, you will receive a thorough clinical examination, and any additional X-rays that are required will be obtained. Your doctor will discuss your diagnosis and your various treatment options in order to best suit your individual needs. Once a treatment plan has been formulated, an itemized estimate of all surgical costs will be provided.

Following the consultation, the reception staff will organize any further appointments for you. This includes surgical appointments at our office, as well as any other referrals that may be indicated. Your consultation appointment is the ideal time to address any concerns or to ask any questions that you may have directly with your doctor. Remember, you can call us at any time to discuss your treatment.

IMPORTANT: A parent or legal guardian must accompany all patients under the age of 18.
